Wellness Program
Most employer groups want to provide "Wellness" to their members but are challenged with how to begin. London Medical Management, Inc. (LMM) has developed several programs to help companies with both short-term and long-term cost containment strategies.
Our health care professionals analyze your health care data to determine the programs that are best suited for your employees. Every employer is different, as are their health care challenges. For example, you may have a large group of members with diabetes, but your wellness initiatives may be more focused on other disease management areas. With our analysis, we are able to identify this and direct your efforts more efficiently.
Here is a value-added benefit that combines our crisis management and claims review expertise with a three-tiered wellness process:
1st Tier A web-based or printed Health Risk Assessment (HRA) form is completed by the member. A copy is sent to both the member and a Patient PAL™ "coach". The coach, a Patient PAL registered nurse calls the member to consult on any results that fall into a moderate or high-risk category.
2nd Tier The member is scheduled for a comprehensive physical with one of LMM's contracted providers, and dollars in the health plan allocated for "preventive health or wellness" are utilized to cover the expense of a basic History and Physical appointment. LMM negotiates with the health plan or providers to cover the cost of the physical.
3rd Tier With the member's approval, the physician who completes the physical shares the results of the exam with the patient's family physician. If the patient is one of the nearly 70% of patients who DO NOT have a family doctor, LMM finds them an in-network physician and schedules a follow up appointment with that physician.
We believe this three-tiered process will engage the member in the health plan and provide preventative services that will, in turn, result in cost savings to the company.
